Executive Summary

This research investigates the frequent failures which are repeatedly occurring in United Energy Pakistan (UEPL) operation zones/facilities. This study identifies fatigue failure (FF) and microbiological influenced corrosion (MIC) which are most frequent failures. Methodology: Data is collected from all operation zones in the form of individual leak investigations, leak incident reports and interviews along with failure part/components specific information.
