Failures are become a hindrance or cause of suspending the operations in any repetitive systems. In upstream oil and gas operations the commodities streams from well reservoirs always contain on deferent and complex composition along with direct interactive with metal surface therefore they react on metals according to the properties of materials and eventually accelerate the different type of corrosion and associate failures which ultimately convert in the form of incidents and production loss/disturbance.
Some time the behavior of attached hardware with piping system also play a major role in occurrence of incidents and/or failures. Therefore the Oil companies which are engage in upstream operation keep more focus and spend the sufficient money to secure their assets/pressure envelops so that they can run their plants/facilities operation at optimum level.
This research study shall investigate the frequent failure (s) which is/ are repeatedly occurring in United energy Pakistan limited, and it shall be contained on two stages, The most frequent failure shall be determined at first stage and the root cause analysis shall be carried out on second stage along with appropriate solutions/control suggestions for mitigation the failures.
